LivWell Community Health Services uses a multidisciplinary approach to care, bringing together medical providers, case managers, and support services to ensure individuals living with HIV receive the most comprehensive care possible. This collaborative model helps improve both the quality of life and long-term health outcomes for people living with HIV.
Our Service Area
LivWell Community Health Services serves a large rural region that includes 26 counties in Western Kentucky and 17 counties in Southern Illinois. These counties are designated by the Health Resources and Services Administration (HRSA) as LivWell’s service area for the Ryan White Part C program. However, LivWell is able to provide medical care to patients who live outside of this designated region.
In addition to our main clinic in McCracken County, Kentucky, LivWell operates monthly satellite clinics in Jackson County, Illinois, and Christian County, Kentucky to expand access to care throughout our service area.
Because case management services are administered at the state level, patients from Illinois and Tennessee may receive medical care at LivWell while working with case managers through their respective state programs.

LivWell Community Health Services Clinic
The LivWell Community Health Services Clinic is home to our primary medical team, as well as our case management, housing, and administrative departments. In addition to LivWell’s services, the clinic building also houses NPS Pharmacy and Infectious Disease Associates, providing convenient access to additional healthcare services for our patients.
The LivWell Community Health Services Clinic is located at 1903 Broadway in Paducah, Kentucky.
