LivWell Community Health Services uses a multidisciplinary approach to provide those living with HIV the most comprehensive care possible. This approach helps to enhance the quality of life and length of life for persons living with HIV/AIDS.
OUR SERVICE AREA
Our vast, rural service area is composed of 26 counties in Western Kentucky & 17 counties in Southern Illinois.
HRSA specifically identifies these counties as LivWell’s service area for Ryan White Part C; however, LivWell can still treat patients who live outside of these counties. In addition to our main clinic in McCracken County, we also travel monthly to Jackson County, IL and Christian County, KY to provide satellite clinic services.
Case management services are state specific; therefore patients from Illinois and Tennessee can see LivWell for medical care, but have case managers based on their state programs.

The LivWell Community Health Services Clinic houses our primary medical team, case management, housing and administrative departments. In addition to LivWell, our clinic building also hosts NPS Pharmacy and Infectious Disease Associates. The LivWell Community Health Services Clinic is located at 1903 Broadway in Paducah, KY.
