EIS Prevention and Outreach Coordinator
The EIS Prevention and Outreach Coordinator supports LivWell Community Health Services’ mission by providing HIV testing, education, and outreach services throughout the community. This role coordinates testing events, builds partnerships with local organizations, and engages diverse populations to promote prevention and reduce stigma. The Coordinator also delivers pre- and post-test counseling, maintains accurate data reporting, and represents LivWell at community events. This position requires strong communication skills, cultural sensitivity, and a commitment to compassionate, non-judgmental care.
Support Services Case Manager
The Support Services Case Manager offers person-centered support while guiding clients through the social services and healthcare systems. The Support Services Case Manager works closely with clients to provide coordination of resources, goal-setting, and establish continuity of care through ongoing evaluation of needs and personal support systems. This position also collaborates with a multidisciplinary team to identify and eliminate barriers to care and treatment adherence.
