The Support Services Housing Case Manager offers person-centered support while guiding clients through the social services and healthcare systems regarding housing stability. The Support Services Housing Case Manager works closely with clients to provide coordination of resources, goal setting, and establish continuity of care through ongoing evaluation of needs and personal support systems. This position also collaborates with a multidisciplinary team to identify and eliminate barriers to care and treatment adherence.
The Housing Liaison serves as a bridge, connecting, and coordinating efforts to ensure smooth operations and efficient decision-making with housing needs. This position plays a key role in supporting access to affordable housing services and resources by offering person-centered support while guiding clients through the social services and healthcare systems regarding housing stability and resource needs. The Housing Liaison works closely with clients to provide coordination of resources, goal setting, and establish continuity of care through ongoing evaluation of needs and personal support systems. Acting as a conduit by building and maintaining landlord relationships. Ensuring continuity of care and optimal health outcomes.

The Support Services Manager oversees the Support Services department operations and collaborates with the Support Services Director to provide guidance to the Support Services team. The Support Services Manager is proactive in creating person-centered departmental protocols to assist HIV-positive individuals in obtaining appropriate support services, and assuring compliance with all programmatic, regulatory, and licensing requirements.
