Our Providers

Dr. Anita Fleenor-Ford joined LivWell Community Health Services as Medical Director and practicing physician in 1998 after relocating to Paducah. She earned a double major in biology and chemistry from Western Kentucky University and received her medical degree from the University of Louisville School of Medicine.

Dr. Fleenor-Ford completed her internal medicine residency and a subspecialty fellowship in infectious diseases in Chicago, Illinois. She is board certified through the American Academy of HIV Medicine.

In addition to her work with LivWell, Dr. Fleenor-Ford is a partner at Infectious Diseases Associates PLLC, which shares office space within the LivWell Clinic in Paducah.

Dr. Carl LeBuhn earned his medical degree from the University of Iowa and completed his internal medicine residency at the University of Iowa Hospitals and Clinics, where he also served as Chief Resident in Internal Medicine.

Dr. LeBuhn went on to complete a subspecialty fellowship in infectious diseases at the University of Iowa Hospitals and Clinics and spent three years teaching in the Division of General Medicine and Infectious Diseases. He is certified through the American Academy of HIV Medicine.

Dr. LeBuhn is a partner at Infectious Diseases Associates PLLC.

Physician Assistant Reagan Poe completed her undergraduate studies at Morehead State University before earning her graduate degree from the University of Kentucky. She began her healthcare career as an X-ray technician and has now served as a Physician Assistant for more than 17 years.

Reagan is certified by the American Academy of HIV Medicine as an HIV Specialist. In addition to providing HIV-focused care, she also serves as a primary care provider for patients, helping ensure individuals receive comprehensive and coordinated medical care.
